This extension damages the content of a plain text files with non-ascii symbols. The encoding of a text file does not matter.
UPD: Thanks to the author for the quick response! I changed the rating to four stars because, taking into account Bug 1954372, I see that the damage to the attachment is not due to an error in the extension.
However, if the bug is known to the author and the author does not know a workaround, users should be warned when saving text files that text files containing non-ascii characters will not be saved properly. Otherwise, users may accidentally lose important data.
P.S. Maybe it will be useful to someone who is experiencing the same problem. Since I can't use Extract 'Em! yet, I save the attachments in two steps as follows: 1) I save all messages with attachments to .eml files. 2) Then I extract attachments from .emls with the munpack *.eml command from the mpack package, which is available in many repositories.
The extension does not modify attachment file content; it simply requests it from Thunderbird and saves the content as it was received. A bug report has previously been filed with Mozilla in this regard; please see https://bugzilla.mozilla.org/show_bug.cgi?id=1954372
