Updating from my previous review, Ruben directed me to the GitHub issues page for the project, and the following instructions fixed my issue (thanks, Ruben!):
1. Go to the Thunderbird Edit -> Preferences -> General and scroll all the way down the page.
2. At the end of the page you should see a button called Config Editor. Click it to open the about:config dialog.
3. Click on the I accept the risk! button.
4. The following strings should be set as follows:
-- media.navigator.permission.disabled true
-- media.setsinkid.enabled true
-- media.allowed-to-play.enabled true
-- media.autoplay.enabled true
-- media.block-autoplay-until-in-foreground true
-- media.playback.warnings-as-errors true
5. Close the window and restart Thunderbird.
This should allow you to give Google Voice permission to access your audio devices, AND you should be able to choose which devices you want to use for each purpose.
One thing to note: Google Voice may not show ALL of your devices when opened through Thunderbird. For example, the output device that I use is a multichannel, output-only USB audio device, and it's not showing up. However, if you go into your Windows audio settings and, in the program specific settings, change Thunderbird to use your desired input and output devices, then Google Voice's settings can be set to "Default audio output device," and it should function as you desire.

Meh. As with all browser tabs in TB once you're logged into one Google account, you can't add other accounts because all it does it open a blank page that goes nowhere. Without that these little buttons are worthless.

It works! I had been trying to get G-hub to work for months and it would always say "something went wrong". This just works. Nice work!
