i think this would be a great tool if it would:
. suppress the "forced" address book name and date (will not stay unchecked)
. if it resolved the page break bug in thunderbird (records split across page end)
. allow for selective field print

1) you're right, it was a bug and it's fixed in 1.0.2.1 version, please install it
2) About the split problem, I could solve it easily if Mozilla fixed this 8-years bug https://bugzilla.mozilla.org/show_bug.cgi?id=132035
Unluckly there is no other way.
3) I will keep in mind this useful feature for future development
