Today suddenly FireFox 6.0.2 on Mac OSX 10.6.8 had become the same problem...
Because I managed to solve it on a WinXP PC I tried my luck in OSX and it worked. :-)
In OSX you can find the needed file in:
\User (your name) \Library\Application support\FireFox\Profiles\RANDOM CODE HERE.default\Extentions\{582195F5-92E7-40a0-A127-DB71295901D7}.xpi

Further the solution is the same for Win and OSX.

First of all Divrsit deserves all the credit!

At home I'm using Firefox 4.x on MAC OSX Snow Leopard without any problems...the add-on never stopped working.

At work I used to using the same Firefox version on Windows XP Pro but with the problem that the add-on was asking to login....without success.

I updated the WinXP FireFox to version 6.0.2 but the problem still exist...?

After reading the feedback here I tried the fix that is described by Xander and it worked for 100%.(https://addons.mozilla.org/nl/firefox/addon/gmail-manager/reviews/310882/)So to all....give it a shot...

I found the file under C:\Documents and Settings\MyUsername\Application Data\Mozilla\Firefox\Profiles\mtrp3we6.default\extensionsFirst I made a copy of {582195F5-92E7-40a0-A127-DB71295901D7}.xpi.....you never know....;-)Then I renamed the xpi extension to zip to open it easily with Winzip.

Next I extracted the contents to a temporary folder on my desktop.Located the necessary file (see explanation of Xander) and replaced what they described.

After that I made a new zip with the reworked contents of the temporary folder and respected the internal dir structure so the file looks the same as the original zip.

Last I renamed the new zip back to xpi and replaced this file with the original in the above mentioned folder.Start Firefox....watch....and smile....Success!
