Major overhaul of the code (thanks to Steven Hartland) to improve reliability. Key improvements:

1. Multiple copy safe.
2. Duplicate message detection which de-duplicates messages caused by multiple runs to Gmail.
3. Error recovery and watchdog support for stalled transfers.
4. More reliable folder creation process.
